Preliminary care and advice from medical professionals are available, for example, care/advice on injuries/use of medicine/reading of blood work result and x-rays.

How to Request for Service

1) Select DETAILS to schedule a Preferred Date.

2) In Google Form, enter information on Date and Time.

3) An email confirmation will be sent to your email account one day before the appointment date.

4) Follow the instructions as specified in the email / Google Form and attend the appointment at the date and time as confirmed.
