School/Major Transfer Request Form

RD.111 SCHOOL/MAJOR TRANSFER FORM AND RD.112 Course/Credit Transfer Request for School/Major Transferred Students 

Required documents to attach as a PDF file with the completed form

         – Attach file – photocopy of the students’s Thai National ID Card or Passport with their signature certifying the accuracy of document.

INSTRUCTIONS

         (1) Download “SCHOOL/MAJOR TRANSFER FORM (RD.111)” and “Course/Credit Transfer Request for School/Major Transferred Students (RD112)”. Complete the form (please print only).

**For RD.112: Please provide information on courses and grades of all subjects you have studied as shown in the academic transcript, excluding information of courses with the letter grade F and/or W. For the current semester, please specify the grades of enrolled courses with the letter grade R.**

         (2) Attach the completed forms and required documents
as PDF files and submit by email at records_service@bu.ac.th

         (3) We required to check the information. When the process is completed, Records Division will send the result by email.
